All-on-4 is a treatment concept developed by the implant company Nobel Biocare. A full arch of replacement teeth, upper or lower, is fixed to four implants placed in the jaw. The two back implants are usually angled, which can make good use of the available bone and, in some cases, avoid the need for bone grafting. The teeth are fixed in place and are not taken out at night like a denture; only the dental team can remove them.
It may suit adults who have lost all or most of the teeth in one jaw, have teeth that can't be saved, or struggle with a loose denture. Suitability depends on the amount and quality of bone, your general health, and things like smoking and diabetes control, which affect healing. Some people are better served by a different number of implants, an implant-retained denture or another approach. You will only know after an assessment.
We examine your mouth, go through your medical history and take X-rays and usually a 3D CBCT scan, so the implants can be planned around your bone, nerves and sinuses. You will be given a written treatment plan with the stages, risks, alternatives and costs before you decide. Where the implants are stable enough when they are placed, a fixed temporary bridge is often attached on the same day or shortly afterwards. This temporary set is worn while the implants fuse with the bone over the following months. The final bridge is then made and fitted. If the implants aren't stable enough for immediate teeth, your dentist will explain the alternative timeline.
Surgery is carried out under local anaesthetic, and sedation can be discussed if you are anxious. Swelling and tenderness are common for several days, and you will be asked to stick to soft foods for the first weeks while the implants settle. Most people plan a few days off work. We give written aftercare instructions and review you at regular intervals.
Like natural teeth, implants can develop gum problems if they aren't cleaned well, and the bridge itself may need repair or replacement over the years.
